Dissertation Abstracts Some dissertations contain original instruments.

PsycINFO - Add the word "appended" to your search and it may pull out some references that say, "test appended" or "instrument appended" indicating that the article contains the test.

MEDLINE - Can be searched using Ovid or PubMed

Sociological Abstracts - Social and behavioral sciences.

Web of Science - May be searched to identify articles that refer to tests or questionnaires.
